1. Images are associated to stylesheets, not templates. The theme packager does include all the style images directory. 2. Hadn't noticed custom templates weren't included, I'll include them in 7.0.9. 3&4. No plans for those since they're not theme-related. As a customization project I could add those options for $50 though.
I believe all of those are are part of the language pack, so if you use the "Add entire new language pack?" option you should get them.
Selecting 'add custom fields' is not supposed to include settings and switches -- there's a separate "Include settings, switches and tweaks?" option for that below it.

I've also noticed that even if you tick 'add custom fields' when you create a theme package, settings and switches custom fields do not get packaged.

Yeah, but if your custom field IS a switch or setting, it is not packaged no matter what you Include.
