I would like to suggest that when adding a new field, to have the possibility of stating it as a required field to be filled in. Same as the actual username and password fields.
Making it multilingual and dealing with custom fields are the big problems there. With custom fields, the mysql field name wouldn't tend to be a user friendly way to identify the field. With multilingual, there'd have to be some sort of big mess involved to have translations of each field name including custom fields.
