At Settings->SEO
Replace ampersands with __ (must be unique)

The character we insert here replaciong the ampersands is not allowing the rewrite to work at the category or link that contains this character.

Naturally, it asks you for something unique because it's logically impossible to have the same thing url mean two different results.
