This isn't Paul, but hopefully I can help answer your questions:
1. If you go to the Functionality link in your Admin section, check your Registration Type. One of the options is Require validation by Admin. Change it to email validation or no validate and that should fix that problem.
3. For some reason you skipped #2 . I'm not sure about how to limit to one email address per account. I didn't see anything that would let you do that.

The answer to 1 put more simply is that it doesn't. That you can validate them as admin doesn't mean you have to.
For #3, I'd have to guess maybe you mean one account per email? Two emails in an account sounds rather illogical to me.
