Do we need to chmod ALL the files inside /attachments/ and /admin/ and /languages/ to 777?

Or we just chmod 777 for the folders?

It means the directories, not files, except where it specifies files.

How about templates? All the files or just the folder?

If it were the directory it would say chmod your /templates/ directory. Also, if you see the manual entry on chmoding, chmoding any directory to 666 would kill it. 666 is always files. It means to chmod the template files within your template set (and the location of your template set, of course, varies depending on which one you're using).
