All uncustomized templates will be replaced with the new versions either way. Without checking the box, customized templates will have changes inserted at appropriate points automatically and instructions given to you for what it can't do automatically. With the box checked, customized templates will just present a change list without applying any of it.
It was removed a year ago for being excessively confusing and leading people to make a choice they didn't intend. The $skipcustomtemplates tweak now does the same thing: scripts.webmastersite.net/w...l/other/tweaksphp-359.html . Odds are you don't want to use it. It creates more work for you than not using it. Customized templates are not overwritten during upgrades (from any 5.0 or later version), just lines of them which haven't changed from the old default are automatically modified to the new default to save you the trouble of doing that yourself.

From what version? Upgrading from 4.1.0 or higher doesn't wipe templates.

When autoupgrading you get a check box to tick if you do not want upgrading to auto upgrade your custom templates.
