I changed category permissions for just one category, so that guests, members and sponsors could not rate links, submit links, submit comments, submit categories. I did not touch permissions for admin or mod, so they would just be the default options.
However, nobody can now view the category, whether as guest or logged in as admin
I entered the exact configuration described for those three groups on a category, but the category was still visible acting as normal for me after that.
Check the "Default to no access?" value on the category. The only theory I can see to explain what you're seeing is that "Default to no access?" is set to "yes" on that category. If it's set to "yes" then the category will be inaccessible until all the "on" columns are manually selected.
