There's never been any instruction that discusses upgrading to any version other than the one which the readme file is contained in -- that would be a bit silly, it's impossible to upgrade to a version other than the one the files you've downloaded are of. It says "TO UPGRADE FROM 4.0.10 - 4.0.19", not "to".

The only install instructions in the README.html file is from a previous version TO 4.0.19. The latest version is 4.0.20.
Did you forget to update the README.html file, or did you put up a wrong download link?
