You can't add fields to reports. The reporter's data is already available just by using {MEMBER variables though, I believe, if the reporter is a registered member. I'll add some documentation of the report template vars.

On the report form I would like to add the input fields for User ID (if applicable) and Email so that I know who is reporting the problem.
I guess this should be simple but am not sure how it works. Please tell me the fields I need to add. Thanks.
