Comments on formemail and form fields

My problem is that emails are sent but some fields are missing. For now i have subject and sender email, but i don't have the message itself nor the other fields i added. Could someone help me on this one ? Thanks.

I followed the instructions, but i guess i missed something. I still don't get why i don't have some fields. I still get the subject and the sender's email but nothing else. Could you please help.
